The program aims to improve quality of life, health and wellness by identifying upstream and downstream opportunities. The program provides holistic, dynamic and unique community circles of care for each person based on the important activities, opportunities, challenges and unmet needs they identify. This program is designed for those feeling lonely and wanting to do more of what they love. Clients set goals, plan action and create the opportunities that you want for themselves. The Windsor-Essex Compassion Care Community (WECCC) is a long-term population health research program to build connection. WECCC is bringing community, volunteers and care teams together to help individuals and families achieve their goals; address physical, mental, social and spiritual needs; create social opportunities; and track quality of life. WECCC is an initiative being run under the authority of Hospice of Windsor-Essex as its lead agency.
