Description of Services: | Daytime relief for caregivers who are living with or in the process of obtaining a diagnosis of Alzheimer's disease or other progressive dementia * clients are matched with a respite companion that provides social visits to the home * visits are a minimum of 3 hours at regularly scheduled times, up to 20 hours per week | ||

Fees: | $8.50 per hour * subsidies may be available | ||
Eligibility - Population(s) Served: | Adults with memory impairments (Alzheimer's disease and related dementias) living in the community, and their family caregivers * must have a valid OHIP card * must be independent with toileting * must not require physical assistance with transfers or lifts * must be able to take medication independently * must not have been considered eligible for crisis placement in a long-term care home by Ontario Health atHome | ||
