Program: Niagara Health at Home (NHAH)

Description of Services: 16-week homecare program that offers an individualized service plan whereby individuals receive at least two (but can be more) of the following types of services: personal support worker, occupational therapist, physical therapist, nursing, social worker, and registered dietitian * services are provided through Bayshore for patients living in St Catherines and Thorold and SE Health for patients living in south Niagara

Eligibility - Population(s) Served: Patients who are medically appropriate for discharge who require 16 weeks or so of homecare supports to be able to meet their medical, personal care and rehabilitative need
